After School Activities Booking Form

This is the form to book your child's places in our after school activities clubs, for the summer term 2021 .
Activities run each Tuesday, Wednesday and Thursday during term time, and we are now taking bookings for the term below. Please complete the form ensuring that for clubs where there are limited places, you have specified a second choice for your child as well.
